Preparing to Fill Out Forms

Make sure the person changing their name qualifies.

  • The person changing their name must have lived in the county where the petition is going to be filed for at least two consecutive years.
  • The person changing their name must be 18 years of age or older. Or have their parent(s) or legal guardian(s) file the petition for them.
  • The name that the applicant is changing to cannot be a curse word, racial slur, or something considered obscene or offensive.
  • Please be aware - Name changes for minors under the age of 18 can be tricky. The permission of both parents is not required. However, a judge is more likely to allow a name change when both parents have agreed.
  • Please be aware - You cannot change your name to avoid legal actions. Also, you cannot change your name to illegally get money as someone else.

Before filling out our forms you will want to gather the following pieces of information. These forms can take a little time to fill out. We recommend setting aside an hour or two for this process and having all the information by you when starting.
  • You will need the following information for the person changing their name:

  • Current and new name
  • Contact phone number
  • Home address (street address, city, state, and zip code)
  • Wyoming county they live in (as well as the approximate date when they moved to the county)
  • The city, county, and state that the person changing their name was born in
  • A short (one to three sentence) explanation of why the person wants to change their name. This should be a reason other than just preferring the new name.
  • We recommend filling out all the forms you need at one time.
They will not be filed at the same time, but you will have everything you need to do the next step of the process when the previous one is complete.
